For a student to participate in athletics at Cypress Christian School, the following forms must be completed and returned to the Athletic Department prior to the first practice. Do not give forms to the athlete's coach.
One parent AND the player must read and sign the code of conduct in the Athletic Section of the CCS Combined Policies Handbook, the Acknowledgment of Rules for middle school or high school, Athletic Physical form and Transportation Waiver.
Proof of Medical Insurance for Students
Please note that the school will require proof of medical insurance coverage in addition to a sports physical and other athletic department forms prior to a student participating in the school sports/co-curricular activities. Please log in to FACTS (formerly RenWeb) Parent's Web and go to School / Web Forms / Current Optional Forms / Athlete Insurance Information and enter your insurance company name, policy number and expiration date. A copy of your insurance card is no longer required.

The Texas Association of Private and Parochial Schools (TAPPS) is excited to announce a partnership with Rank One. The company will begin handling TAPPS student and school personnel registration eligibility beginning in the 2019-2020 school year. The partnership with Rank One will transition TAPPS forms and their submission to a more electronic format as the year begins.
Parents will be able to use the app to check eligibility, see the game schedules with directions for away games, view practice schedules (most accurate info will be sent by the coach on a weekly basis) and receive email notifications of game, date, time or location changes. Once the districts are linked, you
should be able to see other school results to check district standings.
