2023-2024 Athletics Forms
The Texas Association of Private and Parochial Schools (TAPPS) has partnered with Rank One Sport to handle TAPPS student eligibility. All TAPPS and CCS high school and middle school athletic forms must be submitted to this electronic format. Upload all student physical forms to Rank One.
Parents will also be able to use Rank One to check student eligibility, see game schedules and receive email notifications of game, date, time or location changes.

PLEASE NOTE: All athletics forms must be completed in Rank One. Students must have a current physical and insurance information must be completed in order to participate. If student received a physical on campus, the physical form has been uploaded to Rank One.

Houston Methodist Offering Sports Physicals on Campus

Houston Methodist Orthopedics & Sports Medicine will provide sports physicals for secondary students who will be in grades 6-12 for the 2023-2024 school year.
- Wear light, comfortable clothing
- Bring glasses or wear contacts if you have corrected vision
- Bring your TAPPS physical form, medical history form and Methodist consent form completed

Required Student Medical Insurance
Please note that the school will require proof of medical insurance coverage in addition to a sports physical and other athletic department forms prior to a student participating in the school sports/co-curricular activities.
- Log in to FACTS Family Portal and go to School / Web Forms / Family Demographic Form
- Click on Student Medical Form button next to your child's name
- Go to the Physician and Insurance section
- Add or update your insurance company name, policy number and expiration date
- Select Yes in the Permission to treat field
- Click Save at the bottom of the page
A copy of your insurance card is not required.
Accident Insurance Information
CCS has partnered with Texas Kids First Insurance to offer primary or supplemental medical accident insurance as may be desired or needed by individual families. This partnership will make a student accident insurance plan available to school families that choose to participate, should they need and desire coverage or supplemental coverage for their children. Texas Kids First provides this type of coverage to families in over 350 private and public school districts throughout Texas.
