Forms

For your son or daughter to participate in athletics at Cypress Christian, the following forms must be completed and returned to the Athletic Department prior to the first practice.  Please turn them in to Cindy Wilhite and do not  hand them to your student's coach.  Both one parent and the player must read and sign the code of conduct in the Athletic Handbook, the Acknowledgment of Rules for MS or HS, Athletic Physical form and Transportation Waiver.
